During a breast reduction, excess breast tissue, fat, and skin are removed to create a smaller, lighter, and more lifted breast shape. Because the breast is reduced and reshaped, the nipple and areola usually need to be repositioned higher on the breast to achieve a proportionate result.

Does the Nipple Get Removed During Breast Reduction Surgery?

In most breast reduction procedures, the nipple remains attached to the underlying breast tissue through a structure known as a pedicle, which preserves its blood supply, nerve supply, and, in some cases, the ability to breastfeed.

In the vast majority of breast reduction surgeries, the nipple is not completely removed. Instead, it is carefully moved to a higher position while remaining connected to the breast tissue.

This technique helps maintain:

• Blood flow to the nipple

• Nipple sensation

• Areolar appearance

• Potential breastfeeding function

Your plastic surgeon will choose the most appropriate technique based on your breast size, anatomy, and surgical goals.

What Is a Free Nipple Graft?

In some cases involving extremely large or significantly drooping breasts, a free nipple graft may be recommended.

With this technique:

• The nipple and areola are completely detached

• Excess breast tissue and skin are removed

• The nipple is grafted back onto the breast in a higher position

While this can be a safe and effective option for selected patients, it generally results in:

• Permanent loss of breastfeeding ability

• Reduced or absent nipple sensation

• Potential changes in nipple pigmentation

Fortunately, free nipple grafts are very uncommon and are usually reserved for specific situations.

Will My Nipples Be Smaller After Breast Reduction?

Many patients choose to have the areola reduced in size as part of their breast reduction surgery to achieve a more proportionate result.

Large breasts are often associated with enlarged areolas, and resizing the areola can help create a more balanced breast appearance. Your surgeon will discuss the desired size and shape during your consultation.

Will I Lose Nipple Sensation From Breast Reduction Surgery?

Changes in nipple sensation are common after breast reduction surgery.

Immediately after surgery, you may experience:

• Numbness

• Reduced sensation

• Increased sensitivity

• Tingling sensations during healing

For most patients, sensation gradually improves over several months as the nerves recover. However, some degree of permanent change in sensation can occur.

The risk of long-term changes depends on factors such as:

• The amount of tissue removed

• Surgical technique used

• Individual healing characteristics

• Nipple sensation before surgery - some patients with very large breasts don’t have much feeling in the nipples to begin with, even before any surgery

What Do the Nipples Look Like After Breast Reduction?

Following breast reduction surgery, the nipples are typically:

• Positioned higher on the breast

• More centred on the breast mound

• Better proportioned to the breast size

• More symmetrical

Initially, the nipples and areolas may appear swollen, slightly uneven, or sit higher than expected. As swelling settles and tissues soften, the final position becomes more apparent over several months.
